When you request that we change your name on our records, you will need to provide us with:

Documentation
You will need to upload a document to confirm your name change is official from one of the following types:
- Marriage Certificate
- Decree Nissi (in the event of a divorce)
- Property Title Extract showing new name (Extract needs to be within the last 30 days)
- Birth Certificate
- Australian Business Number (ABN)
- Australian Company Number (ACN)
- Certificate of Incorporation
You will be required to certify that all details provided are true and correct.
