Care Packages Program
The Home Care Package (HCP) Program helps older Australians to live independently in their own home for as long as possible.
There are four package levels which are structured as follows:
- Level 1 supports people with basic care needs
- Level 2 supports people with low-level care needs
- Level 3 supports people with intermediate care needs
- Level 4 supports people with high-level care needs.
Each level of home care package provides a different subsidy amount, determined and paid for by the Federal government. This amount is held by the government and your provider will use that fund at the end of each month to pay for your services and supports.
The money provided in your HCP contributes to the total cost of delivering care to meet your individual needs. It is also expected by the government that you will contribute to the cost of your care where your personal circumstances allow.

There are many Home Care Package Providers in the Greater Geelong region. We encourage you to use the Find A Provider function on the My Aged Care website to find a suitable provider.
Quality of life
Living at home is much more than just keeping your house clean and tidy. Our service aims to enrich your life and draws upon an extensive list of services through our preferred provider network, for example:
- Home visiting services – Podiatry, Occupational Therapy, Physiotherapists
- Support Services – showering and dressing, home care, social support, respite, shopping assistance
- Orthotics
- Basic Gardening services to keep the yard safe and maintained
- Transport
- Food preparation
- Aids and equipment assistance – Walking sticks, shower chairs, handrails
- Planned activity groups and socialisation programs – Men’s shed
Our Care Managers are
experts in helping our HCP clients to understand how to use their HCP support appropriately to maximise independence and support to live at home – or similar.
Fees
We do not charge a basic daily care fee for services provided unless a client is exceeding the funding they are provided within their allocated Home Care Package. However, a client may need to pay an income tested fee as established by the Centrelink income assessment.
Eligibility
To determine if you or a loved one is eligible for a Home Care Package the steps are:

Contact My Aged Care on 1800 200 422 or via the accessing home care page on the My Aged Care
website. This is the only way to enter and register for a Home Care
Package.
- An assessment will be
organised by My Aged Care and the ACAS team
- If eligible, there can be a
wait time until a Home Care Package becomes available.
- When a home care package
becomes available you will receive a letter from My Aged Care to let you
know you have been assigned a package. The letter will set out the level of home care package that
has been assigned to you and include a unique referral code.
- With this referral
CODE you can contact The City Of Greater Geelong and discuss your options
to be cared for by our team of experienced staff and to see if we are able to provide your package.
