When a food or health business is being sold, sometimes the prospective owner wants a report of the business under the Food Act or the Public Health and Wellbeing Act.

The written consent of the current proprietor/s is needed to release information to any prospective purchasers.

When you receive the form

  1. Print the form out and complete all the information
    Please read the form carefully, as incorrectly completed forms may delay your request.
    It must be signed by the current proprietor/s.
  2. Return the completed form with your payment
    The applicable fee can be found at the top of the form. This fee includes the transfer fee, so no further payment is necessary as part of the transfer.
    The responsibility of payment of the fee is arranged between the two parties.
    If the sale of the business does not proceed you or your client may be entitled to a partial refund.
    Make payment. The fee can be made by completing the credit card authorisation form at the bottom of this page, or at one of our Customer Service Centres or by cheque in the mail.
    The form can be emailed to contactus@geelongcity.vic.gov.au or faxed to us or dropped into any Customer Service Centre.

What we will do

When we receive the form we will contact the current proprietor to arrange a time to inspect the premise.

We will conduct a full assessment of the business under the relevant Act and provide both parties with a copy of the report, together with a letter which contains important information about the registration. The formal transfer of registration application form will be included with this report.

This process usually takes between 10-14 working days from the date we receive the form and the correct payment.


Urgent requests

If you need an urgent report, you can make a request for an Environmental Health Officer (EHO) to attend the business within two working days. This is referred to as 'Fast track request'. You must let us know you want this at the time you request the form.

We will make every attempt to accommodate all fast track requests, however this depends on the availability of the EHO. We will let you know if your fast track request can be met.
