Hearths home and community support services include:
- assistance with daily living
- personal care assistance
- complex medical assistance
- companionship
- independent living – meal preparation, light domestic duties, travel training
- manual handling – mobility assistance, aids & equipment use
- community engagement.
Every participant, client and support worker has a personal, dedicated single point of contact, within Hearth which we call a Relationship Manager at no cost to the participant.
