A service to connect NDIS participants with other disability services and support providers to be able to meet their goals within their NDIS Plan.
Assistance with formulating and achieving goals, plan reviews, finding and engaging service/support providers, both disability and mainstream providers as well as supporting families/carers in their role as an informal support.
Free support to obtain initial ACCESS to NDIS funding.
An Outreach Service is also available for people experiencing complex situations such as a risk of homelessness, family violence, hospital discharges and for those exiting the justice system.
