The registration form has been designed to provide the basic information required in an emergency.
For example: if someone has an accident, is unconscious or had a stroke and is unable to talk or even if someone is found dead, quick access to the registrant's medical records by listing the name of the doctor and the pharmacist assists with ready access to the registrant's home by recording the location of a house key provides relevant details about pets and their ongoing care if their owner is absent due to an emergency option to request regular telephone calls, newsletters, etc. |
