The aim of the Home and Community Care Program is to provide a comprehensive and integrated range of support services for the frail aged, people with a disability and their carers.
Services are provided to assist people to be more independent at home and in the community and to assist carers in their caring role.
The Program is administered in accordance with the terms of the Home and Community Care Act 1985.

Aged and Disability Services is a department of the Human and Cultural Services Division within the City of Greater Geelong.

To assist the frail aged and people with disabilities to maintain their independence in the community through a range of support services.
YOUR HOME - OUR EMPLOYEES WORKPLACE
Under the Occupational Health and Safety Act, the home where the care of a client is provided is a workplace. It is the responsibility of the occupier of the home to provide, as far as reasonable, a safe working environment for workers coming into their home.

ELIGIBLE CLIENT GROUPS People eligible to receive a HACC service are frail older people, people with physical, functional, sensory, intellectual or psychiatric disabilities, people with acquired brain injury, carers and families living at home in the community. Services are provided on the basis of eligibility and assessed need.
