HOW DO COMMUNITY AGED CARE PACKAGES WORK?

Community Aged Care Packages are designed to assist people who have been assessed as requiring low level residential care and wish to remain living at home but require support and assistance to do so.

THE SERVICE AIMS

  • To provide case management for assistance with the planning and coordination of each individuals complex care needs
  • To give people choices about the services they may need
  • To enable people to obtain support services, equipment and the type of care they need to continue to live at home
  • Packages are flexible and individually designed to meet differing needs
  • To enhance the support given to people by family and friends

HOW CAN COMMUNITY AGED CARE PACKAGES HELP?

  • By developing a package of services for people in close cooperation with the person, family and other service providers
  • By maintaining close contact with the person to ensure their changing needs are regularly monitored and reviewed

TYPES OF SERVICES

  • Case management
  • Showering and bathing
  • Dressing and undressing
  • Meals
  • Arranging social activities
  • Laundry
  • Light housework
  • Home maintenance

ARE YOU ELIGIBLE?

Community Aged Care Packages are available to people who live in the local government areas of Colac/Otway, City of Greater Geelong, Surfcoast or Queensclif and who have been assessed by the Barwon Region Aged Care Assessment Service as appropriate for low level residential care.

The service is provided to those who, in order to stay at home:

  • Have difficulty coordinating services
  • Need more flexible services
  • Who live at home with a carer who needs support

Priority for the service will be subject to assessment.

The City of Greater Geelong is committed to the continued protection of your personal information. We collect your personal information to provide and improve our services to you and to fulfil our statutory obligations. You will be informed at the time of your assessment as to what information is required and how it will be used to meet your needs.

PARTICIPANT FEES

Weekly fees based on the individual's financial situation will be negotiated with the participant, family or representative.
